1. Submit information on left side and click "send". 
2. Fill out the CIAC Background check & information form below in section New Member Requirements and bring to June meeting.
3. Bring a check $15 (rule book) to new member clinic in June. 
4. Pass the rules exam and pay an additional $85.

This fee covers, CIAC background check, Insurance, and yearly dues.
